Personal trainer client questionnaire
A client intake (PAR-Q style health screening plus goals) for personal trainers. It is not medical advice: anyone with a flagged answer should check with a doctor first.
When to send it: Before the first session, every time with a new client.
Health screening
- 1.Has a doctor ever said you have a heart condition or should only do supervised exercise?
- 2.Do you feel chest pain, dizziness or shortness of breath during activity?
- 3.Any injuries, surgeries or joint problems in the past two years?
- 4.Medications that affect heart rate, blood pressure or balance?
- 5.Are you pregnant or have you given birth in the last 12 months?
Goals
- 6.What do you want to achieve, and by when?
- 7.Current activity: what do you do in a normal week?
- 8.Sleep, stress and diet: anything I should know?
- 9.Exercises or environments you enjoy or hate.
Logistics
- 10.Where will we train: gym, home, outdoors, online?
- 11.Days and times you are available.
- 12.Emergency contact name and phone number.
Budget, timing and decisions
- 13.What budget range have you set aside for this?
- 14.Is there a hard deadline, and what happens if it moves?
- 15.Who else needs to approve the work or the contract?
- 16.How would you prefer we communicate (email, calls, messaging), and how often?
- 17.How did you hear about me?
Send it, and get the answers back
Stop copying answers out of email threads. With Clinch the questionnaire goes out with your proposal or contract, your client fills it in from their phone, and the deposit is paid when they sign.

When should a personal trainer send a client questionnaire?
Before the first session, every time with a new client.
Can I edit these questions?
Yes. Copy them, remove what does not apply and add your own. Shorter questionnaires get answered faster: keep only the questions whose answers change your price, your plan or your contract.
Should the questionnaire come before or after the contract?
Questions that affect price and scope belong before the proposal; the detailed ones (names, timelines, access) work best right after signing, when the client is committed and expects them.
How do I send it with Clinch?
Create a free Clinch account, paste the questions, and send them with your proposal or contract. Your client answers from their phone without an account, and the answers are saved on the client's record.
